At Eyecenter Optometric . . .

Our office provides both routine vision as well as medical eye care, and therefore we will need you to provide us with both types of insurance information when you make your appointment. We are participating providers in several major vision and health care plans, including Medicare. Some medical insurances, such as Medicare, have annual deductibles that must be met (a certain amount of cash paid out of pocket for medical services already in the current calendar year) before the insurance plan will begin to cover medical services. Please be aware that it is your responsibility to pay for your services in full if your deductible has not been met. In addition, many plans will cover only part of your fees for services and/or materials. While we understand that this can be frustrating and confusing for patients at times, and while we are committed to providing accurate and concise information about your insurance, ultimately, due to the vast number of different plans, it must become your responsibility to understand what your insurance will and will not cover. Any non-covered fees are due at the time of service. We accept cash, check, VISA, American Express and Discover.

If you have coverage other than those listed below, please feel free to call our office and inquire about our fees, many of our patients simply pay “out-of-pocket” or with a flex-spending account for our services. If you have a reimbursement plan, we will be happy to provide you with a detailed receipt of all of your expenditures.
